Family Needs Assessment
EFMP Family Support can meet with you any time to complete a family needs assessment. The assessment is person-centered, family-focused, and strength-based to support the entire family is services and resources that best meet their needs. The EFMP Family Support office is available to assist in providing training and support to organizations and units in diversity, equity, inclusion, and accessibility to best support our EFMP families. Please call us at 618-256-4746 or email us at 375fss.efmp@us.af.mil.
Per 10 USC Statute 1781, Section 563, Q-coded Airmen must complete or update a Family Needs Assessment/Service Plan with the local EFMP Family Support (FS) Coordinator NLT 60 days prior to a permanent change of station CONUS or OCONUS. An outlook calendar invitation is being sent to you to schedule this mandatory appointment.
We encourage but do not require your spouse and/or family member/s to attend. This is especially important if those family members manage appointments and support services for themselves or the EFMP family member.

Illinois Waiver Services
Prioritization for the Needs of Services (PUNS)
PUNS is a database that registers individuals who want or need Developmental Disability Waiver services funded by the IL Dept of Human Services. Contact your area service coordination agency. PUNS enrollment will help communicate a possible need for services once your child turns 22. It is recommended to enroll at any age as the state uses this data to track future needs.

Air Force Aid Special Needs Grant
Assist eligible Airmen/Guardians who are faced with increased financial requirements due to costs associated with having dependents with special needs.
Eligibility: Active duty, Active Guard Reserve (AGR), Guard/Reserve on an Initial or Continuation Full-time order, and Traditional Guard/Reserve on a long-term order who have dependents enrolled in the Air Force's Exceptional Family Member Program (EFMP).
Grant Amount: Up to $3000 per special needs family member over a five-year period.
